Job Description 

Summary

The CHC mission is to provide high quality healthcare to those who cannot afford it- ministering to body, mind, and spirit. We believe the body, mind, and spirit are intimately connected and injury in any area will affect the others. Consequently, addressing the mental health and emotional welfare of patients is essential to facilitate healing and wholeness.

The counselor’s role is to evaluate and treat patients with mental health disorders or emotional distress through individual and group sessions. The counselor works with other members of the Behavioral Health Team (Social worker, psychiatry partners) alongside the primary care team in an Integrated Behavioral Health Model. The counselor receives internal referrals from CHC providers and BH team members, and collaborates with the entire team to provide the most successful holistic treatment plan for each patient.

Principle Duties and Responsibilities

1) Counseling Sessions

Meet with patients to determine the exact nature of their mental health concerns

Provide targeting counseling designed to help the individual overcome their problems

Assist individuals with implementing behavior modification strategies and coping skills

Remain knowledgeable of the latest evidenced based methodologies in counseling to provide patients with the best possible care

Refer patients to other mental healthcare providers as necessary

Remain warm and engaged through the duration of every patient’s counseling session

Appropriate documentation and billing in Electronic Health Record

2) Team Based Care

Works with the Social Worker to provide crisis management counseling and facilitating in-patient or residential admission for patients who are a harm to themselves or others.

Collaborates and coordinates care with social worker, psychiatry (when applicable), and the primary care team.

Participates in staff morning huddle, weekly staff meetings and training sessions, and leadership meetings as needed.

3) Administrative Responsibilities

Completes charting and billing in a timely manner

Provides supervision for FCHC Social Worker

Assists in writing behavioral health related policies and procedures

Provides behavioral health related staff training

Job Qualifications Education, Skills, and Experience   

  • Master’s degree in counseling or a related field

  • All state (VA)-mandated licenses required for practice as a clinical counselor

  • Knowledge of evidenced based counseling methods (such as Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Acceptance and Commitment Therapy, Family Therapy, interpersonal psychotherapy, etc.)

  • Excellent communication skills

  • Ability to get clients to open up to you (Humble, compassionate, kind)

  • Ability to deliver treatment that is customized to meet the unique needs of every patient

  • Ability to incorporate spirituality into counseling sessions as appropriate/desired

  • Willingness to refer patients to other healthcare providers as needed; works well with team of social worker, psychiatry, and primary care

  • Ability to read, write, and communicate effectively with clients and staff of different cultural backgrounds.

  • Bilingual preferred.

    Must be able to work with individuals with physical or mental challenges

  • Identifies as a local Christian missionary and passionate about being part of a Christian ministry
