Overview

Our vision is to see our patients and community find healing and wholeness in Christ. The CHC provides care to the body, mind, and spirit of all our patients- with 35-40% of care each year being to the underserved.

The CHC partners with several local ministries to provide care to the homeless, immigrants, those overcoming addiction, victims of abuse and human trafficking, and those with unplanned pregnancies. We hope to be a catalyst in uniting the Body of Christ to be the hands and feet of Jesus together, regardless of differences in denomination, skin color, cultural background, and socio-economic status.

Job Title — Primary Care Physician

Job Description

Duties and Responsibilities: Primary Care physician in outpatient only setting.

  • Provide high quality, evidenced based, primary care medicine to patients of all ages.

  • Provide care regardless of insurance status, treating all patients with dignity and respect.

  • Provide care addressing physical, emotional, and spiritual needs to include sensitivity to social determinants of health and cost of care.

  • Perform comprehensive and problem focused history and physical exams.

  • Provide preventative care to include but not limited to: Annual physical/ wellness exams, women’s annual health exams to include pelvic examination, health education/disease prevention, and immunizations.

  • Perform chronic disease management, emphasizing “in-house” management as much as is reasonable in the primary care setting.

  • Treat acute illnesses and injuries as appropriate in an outpatient setting.

  • Order and interpret appropriate laboratory, radiology, and other diagnostic studies and procedures.

  • Plan and initiate therapeutic interventions, medications, referrals, and procedures with appropriate follow up.

  • Provide patient education and counseling regarding diseases, procedures, medications, nutrition, and healthcare.

  • Review test results and respond to refill requests and patient/staff messages within 3 business days, or 24 hours or sooner as necessary for communication marked as “urgent.”

  • Collaborate care with the clinic behavioral health team, dietician, Advanced Practice Providers, and community partners.

  • Function as a supervising physician for APPs and students.

  • Complete appropriate documentation and paperwork for insurance companies, DME providers, Home Health agencies, pharmacies/PA requests, etc. in a timely manner.

  • Share responsibility of After Hours Call Phone equally with other full-time providers. (Physician is responsible for giving patient advice, and treatment if appropriate, over the phone.)

  • Document patient care services efficiently and completely in the electronic medical record.

  • Maintain professional and technical knowledge by attending educational seminars, reviewing professional publications, participating in professional organization, and maintain a valid license as well CME requirements.

  • Must have the ability to both serve and work on a team with persons of diverse educational, socio-economic, religious, cultural and ethnic backgrounds.

  • Must have excellent verbal and written communication skills.

  • Must have the ability to protect confidential and sensitive information.

  • Must have a valid Virginia license (or be eligible at time or hiring).

  • Must not have any disciplinary actions with any state or institutional medical boards, or other reason to be denied Malpractice coverage or insurance credentialing.

  • Demonstrates clinical skills and competencies to include high quality evidenced based medical care, excellent communication, customer service, continuous quality improvement, emotional maturity, conflict resolution, and team building.

  • Must have the physical ability to perform history and physical exams, and efficiently use the electronic health record.

  • Preferably bilingual in Spanish and English.
