Job Title — Primary Care Provider
Job Description
Community Care center has a full-time employment opportunity for an experienced Primary Care Provider (Physician, Nurse Practitioner, or Physician Assistant) to work at our Joy Southfield site. Clinic care includes the delivery of quality primary health care and other health services and continuity of care to patients, making appropriate referrals with other healthcare providers to meet patient needs.
Providers must be able to function effectively as part of a professional team delivering healthcare in a community setting and have demonstrated abilities for working in a multi-cultural setting.
JOB Qualifications Education, Skills, & Experience
Deliver quality primary health care and other health services to designated consumers of outpatient services and provide continuity of care to assigned patients.
Utilize an Evidence-Based Medicine approach wherever feasible.
Provide consultation and health education to patients, family, and community.
Prescribe and dispense medications as licensed.
Make appropriate recommendations and referrals for follow-up with other health services providers to meet the needs of consumers.
Review and respond to lab results, medical records, and other patient results as part of the healthcare team, in a timely manner.
Precepts mid-level providers as necessary, and cosigns notes as required for mid-level providers and all trainees that interact with provider’s patients or during provider’s actual clinic time and utilizes an Evidence-Based Medicine approach wherever feasible.
Performs health care activities as privileged and assigned.
