Overview
Our Health Center was established to help demonstrate Jesus' love for the whole person through healthcare. Most of our patients are in poverty and nearly half are refugees from all over the world who have fled their homeland due to strife or other circumstances not under their control. Our patients speak over 40 different languages, with significant populations from 11 language groups. Our goal is to provide empowering ministries to demonstrate Jesus' love for the whole person to all who come to us.

Our mission is to provide high quality medical care, for the whole life spectrum, to all who need it especially the medically under-served, people in poverty, refugees and immigrants - regardless of insurance status or ability to pay - to demonstrate Jesus’ unconditional love for the whole person.

Job Description
JOB SUMMARY: The Acute Care Medical Provider provides high quality medical care for patients at the Health Center in collaboration with the clinical team. This position is focused on acute care visits at one of our locations.
Essential Job Duties
*CLINICAL*
Improve patient outcomes and deliver quality care within practice areas as assigned.
Provide high quality acute medical care for adult, pediatric, and basic gynecology patients of the Health Center
Follow up with patients regarding lab and imaging results
Offer patient and family centered care
Ensure consistent, cost-effective care
Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction.
Participate in an office call pool for patient phone calls overnight. Work one Saturday morning clinic per month.
Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health
*ADMINISTRATION*
Maintain electronic medical records efficiently and maximize resources
Create a learning plan that includes familiarity with new research and changes to standard of practiceParticipate in quality improvement activities
Attend required staff and provider meetings except when clinical activities prevent such attendance
Maintains current certifications including all mandatory annual and CPR training
Ability to adhere to professional code of ethics, and compliance to Article 28 and FQHC requirements
*TEAM*
Collaborate with interdisciplinary clinical and program teams at the Health Center
Ensure all actions, personal conduct, and communications represent the organization professionally.
Uphold and ensure compliance to all policies/procedures, mission, and organizational values in a highly professional manner always.
Qualifications
FNP (family nurse practitioner), PA (physician assistant), or MD/DO (physician)
Active medical license in New York State
Board-certified
At least 1-2 years of clinical practice preferred
